This site is hosted by General Agent Insurance Network, Inc. ("GAIN, Inc.") Fortier Insurance Services operates the portal required to collect information provided by and pertaining to you. As such, this notice describes the privacy practices of Fortier Insurance Services, GAIN, Inc., and their respective employees, staff, independent contractors and other personnel (collectively referred to herein as "Fortier Insurance Services"). 1. INFORMATION COLLECTED BY Fortier Insurance Services

Get a Quote Service: When Subscribers utilize our Get a Quote service, we collect non-personally identifiable information about our Subscribers, including zip code.

Provider Registration: When a Provider registers to use this web site, we collect information about that Provider over the web site, including their doctor number, password and/or location number.

Provider's Request of Member Eligibility: Within the Provider area of our web site, Providers can send us requests for member eligibility, which lets a Provider know whether or not a certain Subscriber is eligible for Fortier Insurance Services services. As part of this process, we collect the plan number, Subscriber name and individually identifiable number assigned to each Subscriber (i.e. social security number) in order to process the Provider's request for eligibility.

Aggregate Information: Fortier Insurance Services collects aggregate, non-personal information about its Subscribers and their usage of this web site and any related services. The web server supporting this web site collects internet protocol ("IP") addresses and/or domain names, but not the e-mail addresses, of visitors. This information is not used to identify any individual Subscriber.

Cookies: Fortier Insurance Services also collects information about your use of the web site through the use of cookies. This information will only be used for legitimate business purposes. Specifically, we use this information to process and determine the Subscriber's eligibility for the particular insurance product being quoted. Personal and medical information collected is to be used specifically for application submission, underwriting and billing purposes. Please be assured that the insurance providers are also obligated to maintain the confidentiality of this information.

Affiliated Organizations and Contracted Entities: Fortier Insurance Services shares your personal information with its and the insurer's affiliated organizations and contracted entities involved in processing your insurance application, servicing your insurance application, coordinating information with the insurer or contacting you. Please be assured that the affiliated organizations and contracted entities may only use your personally identifiable information in providing these services and are contractually obligated to maintain the confidentiality of this information.

Service Providers: Fortier Insurance Services shares your personal information with service providers for enrollment verification, marketing purposes, underwriting functions and the like. Please be assured that the service providers are also obligated to maintain the confidentiality of this information.

Plans: Fortier Insurance Services shares your personal information with Subscribers' health plan, employer or plan sponsor ("plan") in order to perform Roger Fortier 's contractual obligations with these plans to provide accurate and complete insurance applications. Please be assured that these plans are also obligated to maintain the confidentiality of this information.

Required by Law: In cases, where required by law or the court, Fortier Insurance Services may be forced to release information regarding our Subscribers or Providers to permit the investigation of credit card fraud, illegal use of the Internet or some other cases of misconduct on our web site.

Subscriber Contact: Fortier Insurance Services may use your personally identifiable information to contact you about the application process. For example, you might be contacted for information needed to complete or process your application or to inform you of the status of your application.
